Following principles of new primary care model could reduce diagnostic errors

Physicians may be able to eliminate costly medical diagnostic errors and the sometimes devastating effect they have on patients by adopting a patient-centered medical home, a new and evolving model of primary care that calls for some reform and emphasizes comprehensive and coordinated care, according to two clinicians who discussed the model in their commentary in the Journal of the American Medical Association.

Hardeep Singh, MD, MPH, assistant professor of medicine and health services at the Veterans Affairs Health Services Research and Development Center of Excellence in Houston, part of Baylor College of Medicine, and Mark Graber, MD, associate chair of medicine at Stony Brook University Medical Center in Stony Brook, N.Y., highlighted in their writing the five principles of the patient-centered medical home, which are having the right: teamwork; information management; measurement and monitoring; patient empowerment; and safety culture in place. These, they report will reduce the number of diagnostic errors.
